Spieghelian hernia — a rare case of an acute abdomen

Summary

Spieghelian hernia is a rare rupture of the abdominal wall along the linea semilunaris. Due to the fact that the rupture is overlain by the aponeurosis of M. externus abdominis, it is often not recognized in its early stage. In most cases the rupture neck is mostly very narrow and in 24.1% of all cases an incarceration of intestinal parts has ensued which may even lead to the development of an acute abdomen. The first choice therapy will consist in an operative support including the reposition of the hernial content and a possible resection of ischaemic organs as well as removal and sealing of the hernia. Because of the rarity of this sickness we report on a patient in our clinic who was operated as an emergency.

Zusammenfassung

Die Spieghel-Hernie ist ein seltener Bruch der ventralen Bauchwand entlang der Linea semilunaris. Wegen der über dem Bruch liegenden Externusaponeurose bleibt er oft lange unerkannt. In 24,1% der Fälle kommt es, bei der meist sehr engen Bruchpforte, zu einer Inkarzeration von Darmanteilen, die bis zur Entwicklung eines akuten Abdomens führen kann. Die operative Versorgung mit Reposition des Bruchinhalts und eventueller Resektion ischämischer Organe sowie Abtragung des Bruchsacks und Verschluß der Bruchpforte stellen die Therapie der Wahl dar. Wegen der Seltenheit des Krankheitsbildes berichten wir uber einen Fall aus unserer Klinik, der notfallmäßig Wegen einer inkarzerierten Spieghel-Hernie unter dem Vollbild eines akuten Abdomens operiert wurde.
