Zusammenfassung
Anamnese und klinischer Befund
Eine 28jährige Frau wurde notfallmäßig wegen einer Synkope und starker Ruhedyspnoe aufgenommen. Schon zwei Wochen vor Aufnahme waren eine Belastungsdyspnoe und eine Ischämiesymptomatik im rechten Arm aufgefallen. Zeichen einer Phlebothrombose bestanden zu keinem Zeitpunkt. Als Risikofaktoren für thromboembolische Ereignisse bestanden eine Adipositas sowie die zehnjährige Einnahme eines Desogestrel und Ethinylestradiol enthaltenden oralen Kontrazeptivums.
Untersuchungen
Die Blutgasanalyse zeigte eine respiratorische Partialinsuffizienz bei erniedrigter Sauerstoffsättigung, und im EKG fanden sich sowohl Zeichen einer Rechtsherzbelastung als auch eines frischen Hinterwandmyokardinfarktes. Durch eine Pulmonalisangiographie wurde die Diagnose einer bilateralen Lungenembolie gestellt.
Therapie und Verlauf
Einige Stunden nach der erfolgreichen Lysetherapie mit Urokinase fiel ein deutlicher Anstieg der Herzenzyme auf. Bei der transösophagealen Kontrastmittelechokardiographie wurde ein persistierendes offenes Foramen ovale nachgewiesen und die Verdachtsdiagnose paradoxe Koronarembolie gestellt. In der Linksherzkatheteruntersuchung fand sich eine umschriebene Hypokinesie im Bereich der Herzhinterwand bei unauffälligen Herzkranzgefäßen. Die Ischämiesymptomatik im rechten Arm korrelierte mit dem angiographisch nachgewiesenen Verschluß der Arteria ulnaris in Höhe der Ellenbeuge. Überlappend mit der Heparintherapie wurde eine orale Antikoagulation mit Phenprocoumon begonnen und die Patientin anschließend entlassen. Wegen eines leichtgradigen Protein-S-Mangels (53%) reiten wir bei zukünftigen Risikosituationen zu einer Thromboseprophylaxe.
Schlußfolgerung
Gerade weil es keine eindeutigen Kriterien gibt, sollte bei unklarer Dyspnoe auch bei jungen Patienten immer an eine Lungenembolie gedacht werden. Liegt der Verdacht einer paradoxen Embolie vor, kann durch die transösophageale Echokardiographie mit echogebendem Kontrastmittel der Pathomechanismus in der Regel nachgewiesen werden. Bedeutsamer Risikofaktor thromboembolischer Ereignisse sind orale Kontrazeptiva der dritten Generation.
Summary
History and Clinical Findings
A 28-years old woman was admitted with severe dyspnea following a syncope. Two weeks before admission exercise-induced dyspnea and ischemic symptoms of the right arm had been noted. Signs of a venous thrombosis were not detectable at any time during the clinical course. Risk factors predisposing to thromboembolism were obesity and the use of an oral contraceptive containing desogestrel and ethinylestradiol for ten years.
Investigations
The blood gas check revealed a partial repiratory insufficiency with decreased oxygen saturation, and there were electrocardiographic criterias of right ventricular damage and inferior myocardial infarction. Pulmonary angiography demonstrated bilateral lung embolism.
Treatment and Course
A few hours after a successful lysis with urokinase an increase of laboratory markers of myocardial infarction was noted. Transesophageal contrast echocardiography indicated a patent foramen ovale. Presumable myocardial infarction was caused by a paradoxical coronary emboli. The left heart catheterization showed a hypokinetic area of the inferior wall, while coronary vessels were normal. In correlation with the symptoms of the right arm angiography showed an occlusion of the right ulnar artery. Overlapping with heparin treatment oral anticoagulation with phenprocoumon was started and the patient was dismissed. Because of a slight deficiency of protein S (53%) a prophylaxis to prevent thrombosis in risk situations was recommended.
Conclusion
Since there are not typical clinical findings, dyspnea even in young patients should be suspicious of pulmonary embolism. Pathological mechanism of paradoxical emboli can be proven by transoesophageal contrast echocardiography. Oral contraceptives of the socalled third generation are an important risk factor of thromboembolism in young women.
