Zusammenfassung
Die APC-Resistenz wurde zwar erst 1993 entdeckt, sie gilt aber als die häufigste thrombophile Gerinnungsstörung in der europäischen und nordamerikanischen Bevölkerung. In über 90% der Fälle beruht sie auf einer Genmutation des Faktor V, der FV : Q506-Allele. Der Erbträger des heterozygoten Typs hat eine 5- bis 10fache Erhöhung des Risikos, an einer venösen Thrombose zu erkranken; ein Homozygoter lebt mit einem 50- bis 100fach größeren Risiko. Wenn aus der Anamnese spezielle Hinweise auf eine thrombophile Gerinnungsstörung vorliegen, empfiehlt sich vor der elektiven Operation eine hämo-staseologische Analyse. Im Fall des Nachweises der APC-Resistenz wird die gewichtsadaptierte Prophylaxe mit Heparin empfohlen. Im normalen Leben bedarf der heterozygote Erbträger nur bei zusätzlichen Risiko-bedingungen einer Antikoagulation, homozygote Patienten mit thromboembolischer Anamnese sind jedoch langzeitig zu antikoagulieren. Bei einer schweren Stammvarikose erscheint die Sanierung des extra-faszialen Venensystems sinnvoll, um die zusätzliche Gefährdung des Pa-tienten durch die Komplikationen der Venenkrankheit auszuschließen.
Abstract
APC resistance was first discovered in 1993. It is the most frequent cause of thrombophilia in Europe and North America. In more than 90% of cases, the underlying cause of APC resistance is a single point mutation in the factor V gene. Heterozygous patients have a between five- and ten-fold higher risk of developing venous thrombosis, while in homozygous patients the risk is increased up to between 50- and 100-fold. Where there is a previous history of thrombosis, a careful investigation of the coagulation system, in particular a determination of the APC ratio, is recommended prior to surgery. In patients with APC resistance, a body-weight-specific thrombosis prophylaxis with heparin is recommended. Heterozygous patients only need anticoagulation when additional risk conditions are present, while homozygous patients with thromboembolic disease should be anticoagulated over a long period of time. Primary varicosis of the saphenous system should be treated promptly by surgery to minimize the additional risk of thrombotic complications.
