Zusammenfassung
Beschwerden durch eine chronische Obstipation sind ein häufiges und oft langfristig bestehendes Problem von Patienten in der Kinderarztpraxis und der kindergastroenterologischen Sprechstunde. Zumeist handelt es sich um die so genannte “habituelle” Verstopfung, die zu der Gruppe der funktionellen gastrointestinalen Beschwerden gehört. In der initialen Evaluation liegt der Schwerpunkt auf Anamnese und Untersuchungsbefund. Wichtig ist das frühzeitige Erkennen von Alarmsymptomen zur differenzialdiagnostischen Abgrenzung von organischen Ursachen. Zur Abklärung werden serum- und stuhlanalytische Verfahren eingesetzt, rektoanale Manometrie und Kolontransitzeitbestimmung ergeben funktionelle Befunde. Rektumschleimhautbiopsie und Kolonkontrastmitteleinlauf haben ihren besonderen Stellenwert in der Diagnose des Morbus Hirschsprung. Zur Beurteilung möglicher Pathologien des Rückenmarks bzw. der lumbosakralen Wirbelsäule ist die Kernspintomographie Methode der Wahl. Manche Patienten benötigen eine kinderpsychologische bzw. kinderpsychiatrische Evaluation. In einigen Fällen kann nur eine stationäre Beobachtungsphase den Einstieg in eine ausreichende diagnostische Beurteilung und erfolgreiche Therapie ermöglichen.
Abstract
Constipation is a common and often chronic problem in childhood. In far most of the cases pediatricians take care of patients with the so-called “functional constipation” which is one of the functional disorders of childhood. In the primary evaluation a thorough medical history and a complete physical examination is usually sufficient. Certain symptoms may indicate an organic reason for the complaint and should accelerate differential diagnosis. The approach is stepwise and comprises serological und stool tests. Anorectal manometry and colonic transit time determination will give functional information. In the diagnosis of Hirschsprung disease rectal biopsy and barium enema are of importance. An MRI of the lumbosacral spine is indicated in cases of suspected intraspinal or spinal pathology. A psychological evaluation will be needed in a subgroup of patients. Sometimes only an inpatient observation enables an adequate diagnostic work-up that results in successful therapy.
