Zusammenfassung
Ein 22jähriger Patient mit bekannter juveniler Dermatomyositis stellte sich wegen plötzlich aufgetretener Muskelschmerzen im rechten Oberschenkel mit begleitender Rötung und Schwellung, einer rechtsbetonten Parese der Oberschenkel- und Hüftmuskulatur und einer Temperaturerhöhung bis 40°C vor. Im Labor fanden sich erhöhtes C-reaktives Protein und Fibrinogen ohne Leukozytose, im Differentialblutbild eine relative Lymphopenie. Die Glutamat-Oxalacetat-Transaminase und die Aldolase waren leichtgradig erhöht. Röntgenaufnahmen der Oberschenkel zeigten ausgedehnte Kalzifizierungen. Im Kernspintomogramm stellten sich ausgeprägte entzündliche Veränderungen im rechten Oberschenkel dar, die durch eine Immun-Entzündungsszintigraphie mit 99mTc-markierten Granulozyten als Phlegmone identifiziert wurden. Am 10. Behandlungstag wurde ein Staphylococcus aureus in der Blutkultur angezüchtet. Die differentialdiagnostische Abgrenzung einer bakteriellen Weichteilinfektion von einem myositischen Rezidiv bei Patienten mit juveniler Dermatomyositis und Kalzinose der Muskulatur kann schwierig sein. Die Wertigkeit konventioneller bildgebender Verfahren ist begrenzt. Die Immun-Entzündungsszintigraphie kann vorwiegend granulozytäre und lymphozytäre Infiltrate differenzieren.
Summary
A 22-year-old male with juvenile dermatomyositis presented with fever up to 40°C and acute pain in his right thigh accompanied by muscle weakness, a skin rash and a tender swelling. Serum aspartate aminotransferase (AST) and aldolase were mildly elevated. C-reactive protein (CRP) and fibrinogen were markedly increased. The differential white blood cell count revealed relative lymphopenia. Radiography showed diffuse calcifications particularly around the thighs and knees of both legs. Magnetic resonance imaging (MRI) demonstrated inflammatory infiltrates in the right thigh. The lesions were identified as phlegmone by immunoszintigraphy with 99mTc-labelled antigranulocyte antibodies. On the 10th day of treatment Staphylococcus aureus was cultured from blood. Patients with juvenile dermatomyositis and calcinosis may develop bacterial infections of soft tissue which sometimes mimic a disease flare. For differential diagnosis plain radiographs, CT scans and MRI are of limited value. Immunoszintigraphy is able to differentiate between infiltrates caused by granulocytes and lymphocytes.
