Abstract

The history of open surgery for groin hernia has gone through many stages of development, including the ancient era (ancient times to the fifteenth century), the era of the start of herniology (fifteenth to seventeenth centuries), the anatomic era (seventeenth to nineteenth centuries), the era of repair under tension (nineteenth to mid-twentieth century), and the era of tensionless repair (mid-twentieth century to the present). Five principles of modern hernia repair developed through these periods of development: antiseptic/aseptic hernia operation, high ligation of the sac, tightening of the internal ring, reconstruction of the posterior inguinal floor, and tensionless repair. Interestingly, many of the initial attempts at laparoscopic hernia repair did not adhere to the recognized principles of hernia surgery learned from open surgery. It is only when the transabdominal preperitoneal mesh repair and the totally extraperitoneal approach, which adhere to the basic principles, are considered that the results of laparoscopic hernia repair procedures can improve and the recurrence of hernia decrease.

Résumé

L’histoire de la chirurgie traditionnelle pour hernie de l’aine a traversé plusieurs étapes: l’ère ancienne (jusqu’au 15è siècle), l’ère du début de la herniologie (15è au 17è siècles), l’ère anatomique (17è au 19è siècles), l’ère de la réparation sous tension (19è au milieu du 20è siècle) et l’ère de la réparation sans tension (milieu du 20è jusqu’à présent). A travers ces périodes, cinq principes différents ont vu le jour: opération de hernie antiseptique/aseptique, ligature haute du sac, rétrécissement de l’orifice interne, reconstruction du plancher inguinal postérieur et réparation sans tension. Il est intéressant de constater que beaucoup des interventions initiales sous laparoscopic n’ont pas adhéré aux principes reconnus, appris tout au long de la chirurgie traditionnelle pour hernie inguinale. Ce n’est que depuis qu’on pratique les techniques de cure par voie d’abord laparoscopique transperitoneale et totalement extraperitoneale que les résultats de la laparoscopic ont commencé à s’améliorer et que le taux de récidive a baissé.

Resumen

La historia indica que la cirugía abierta de la hernia inguinal ha transitado por muchas etapas de desarrollo, incluyendo la era antigua (desde la Antigüedad hasta el siglo XV), la era del comienzo de la herniología (siglos XV a XVII), la era anatómica (siglos XVII a XIX), la era de la reparación bajo tensión (siglo XIX a mitad del siglo XX) y la era de la reparación libre de tensión (desde mitad del siglo XX al presente). A través de estos periodos se desarrollaron cinco principios de ía moderna reparación quirúrgica: la operación antiséptica/aséptica, la ligadura alta del saco, el estrechamiento del anillo inguinal profundo, la reconstrucción del piso posterior del canal inguinal y la reparación libre de tensión. Es interesante que muchos de los primeros intentos de reparación laparoscópica no adhirieron a los principios establecidos con la experiencia de la reparación abierta. Fue sólo cuando comenzó la realización de la reparación transabdominal con malla y la técnica totalmente extraperitoneal, procedimientos que sí adhieren a los principios básicos, que los procedimientos laparoscópicos de reparación lograron mejores resultados y menores tasas de recurrencia.
