Zusammenfassung
Hintergrund
In der Gefäßchirurgie werden zunehmend auch ältere Patienten behandelt. Für dieses Kollektiv sind angepasste Instrumente zur perioperativen Risikoabschätzung nicht etabliert, ebenso wenig wie spezifische flankierende Maßnahmen, um einen erfolgreichen Verlauf für diese vulnerable Patientengruppe sicher zu stellen.
Ziel der Arbeit
Diese Arbeit soll einen Überblick über das geriatrische Assessment geben, dessen wichtigsten Bereiche hervorheben und den Nutzen für die perioperative Risikostratifizierung in der Chirurgie vermitteln. Anhand von Studiendaten und Erfahrungen aus der Arbeit in alterstraumatologischen Zentren werden in der Praxis umsetzbare Empfehlungen für den Einsatz des geriatrischen Assessments und den daraus zu ziehenden Konsequenzen auch für die Gefäßchirurgie gegeben.
Material und Methoden
Selektive Literaturrecherche bis einschließlich Juni 2022 über Medline unter Einbezug nationaler und internationaler Leitlinien mit unterschiedlichen Kombinationen der Suchwörter „geriatric assessment“, „vascular surgery“ und „frailty“.
Ergebnisse und Schlussfolgerungen
Obwohl das geriatrische Assessment die Komplexität multidimensionaler Einschränkungen bei multimorbiden Patienten für die perioperative Risikostratifizierung abbildet und prognostisch aussagkräftiger ist als eine klassische organspezifische Risikostratifizierung, wird es selten in der Chirurgie eingesetzt. Für den Bereich besteht kein Konsens über die Auswahl der Dimensionen und Instrumente des geriatrischen Assessments oder über Konsequenzen für die Praxis. Die Autorinnen empfehlen ein zweistufiges Verfahren: (1) Screening auf geriatrischen Behandlungsbedarf, (2) ein reduziertes Assessment, mit den Dimensionen Selbstversorgungsfähigkeit, Mobilität und Kognition. Aus den Assessmentergebnissen folgen Konsequenzen wie Prähabilitation, Delirprävention und Berücksichtigung des Ernährungsstatus.
Abstract
Background
The proportion of older patients presenting for vascular surgery is increasing. Perioperative risk assessment tools adapted for this population are not established. The necessary specific accompanying measures for this vulnerable patient group to assure successful outcome after vascular surgery are not widely applied.
Aim
The purpose of this work is to provide an overview of the geriatric assessment, to highlight its key areas, and to demonstrate its value for perioperative risk stratification. Based on study data and personal experiences, primarily in geriatric trauma centers, recommendations for the use of geriatric assessment tools are given. Finally, appropriate perioperative measures and treatment strategies for geriatric patients presenting for vascular surgery are recommended.
Material and methods
A selective literature search until June 2022 was carried out via Medline, including national and international guidelines. Different combinations of the keywords “geriatric assessment”, “vascular surgery” and “frailty” were used.
Results and conclusions
Although the geriatric assessment reflects the complexity of multidimensional limitations in multimorbid patients for perioperative risk stratification and appears to have a higher prognostic value than conventional organ-specific risk stratification, it is rarely used in surgery. Beyond geriatric care settings, there is no consensus on the selection of dimensions and instruments for geriatric assessment or on the implications for the practice. The authors recommend the implementation of a two-step process: (1) screening for patients with the need for geriatric management, (2) a reduced assessment with the dimensions self-care ability, mobility, and cognition. Consequences, such as prehabilitation, delirium prevention, and consideration of the nutritional status can then be derived from the assessment results.
