—Ein Fallbericht und Übersicht über die Literatur

Summary

The rupture of the peroneal tendon often causes functional problems and persistent lateral ankle pain. The rupture is often overlooked or misdiagnosed. The acute form is often based on a trauma, while the chronic rupture develops slowly. A dislocated os peronaeum can be seen in some cases on conventional X-rays. The MRI is the diagnostic tool of choice. Patients with minor functional problems should be treated non-operatively. In symptomatic patients operative treatment should be performed. Direct reconstruction should be achieved. In older ruptures the tendons are often retracted, which makes direct reconstruction impossible. In such cases there are such alternatives as: tendon transfer, tendon transplantation or osseous reconstruction. Coexisting pathologies as a rupture of the retinaculum mm. peronaeii sup. or instability should also be addressed. We present a case of the rupture of the m. peronaeus longus tendon and give an overview of the literature.

Zusammenfassung

Die Ruptur der Sehne des M. peronaeus longus wird trotz der Schmerzsymptomatik und deutlichen Funktionseinschränkungen oft verspätet diagnostiziert. Bei der akut auftretenden Form liegt meist ein Trauma zugrunde, die chronische Form entwickelt sich dagegen schleichend. Das konventionelle Röntgenbild zeigt bei vorhandenem Os peronaeum und davon distal gelegener Ruptur dessen Dislokation. Der Goldstandard zur Diagnosestellung ist das MRT. Bei Patienten mit geringer funktioneller Beeinträchtigung ist die konservative Therapie zu empfehlen. Höhergradige funktionelle Beeinträchtigungen sollten chirurgisch therapiert werden. Dabei ist die direkte Rekonstruktion anzustreben. Bei älteren Rupturen ist die direkte Rekonstruktion aufgrund der Retraktion der Sehnenstümpfe meist nicht möglich. In diesen Fällen kann ein Sehnentransfer, Sehnentransplantation oder die ossäre Verankerung am lateralen Os calcanaeum erforderlich werden. In gleicher Sitzung sollte eine zusätzlich bestehende Ruptur des Retinaculum Mm. peronaeorum sup. rekonstruiert werden. Wir berichten anhand eines Patienten über die Ruptur der Sehne des M. peronaeus longus und geben eine Übersicht der aktuellen Literatur wieder.
