Disturbed Sleep and Postpartum Depression

Abstract

The perinatal period introduces a myriad of changes. One important but often overlooked change is an increased reporting of sleep disturbance. Although casually regarded as a consequence of pregnancy or postpartum, there is emerging evidence implicating significant sleep disturbance, characterized by insomnia symptoms and/or poor sleep quality, with adverse outcomes, such as an increase in depressive symptomatology or the development postpartum depression (PPD). Significant consequences may arise as a result including issues with maternal-infant bonding, effective care for the infant, and behavioral or emotional difficulties in the infant. This review discusses the relevant literature as to how disturbed sleep during pregnancy as well as in the postpartum may increase the risk for PPD.

In this paper, the authors prospectively evaluated the relationship between subjective changes in both sleep and biological rhythms and worsening of depressive symptoms from pregnancy to the postpartum period in women with and without mood disorders. They reported that changes in biological rhythms across the perinatal period, but not sleep quality, predicted worsening of depressive symptoms in both groups.

In this groundbreaking pilot study, the investigators measured changes in circadian phase and phase angle between salivary dim light melatonin onset (DLMO) and sleep onset across the perinatal period, and prospectively examined associations between circadian measures and depressed mood in women with a history of major depressive disorder (MDD). They found new mothers had changes in circadian phase position and phase angle between DLMO and bedtime from 3rd trimester of pregnancy to 6 weeks postpartum. They suggest that circadian timing during pregnancy and in the early postpartum weeks may be associated with depressive symptoms.

This paper explores the psychological impact of sleep during the perinatal period on a woman’s ability to cope with motherhood. It addresses not only emotional mood, but the functional aspect of pregnancy. They found that self-reports of habitual poor sleep quality, and specifically, impaired daytime dysfunction due to poor sleep were associated with a low perceived ability to cope practically and emotionally with motherhood-related issues as well as with negative expectations about the future.

J.Psychiatr.Res. 2015. In this recent publication, the authors examine contributors to suicidal ideation in the postpartum. This is important, because suicide is one of the leading causes of death in postpartum women. Identifying modifiable factors related to suicide risk in mothers after delivery is a public health priority. In addition to the usual suspects, physical abuse and anxiety, they found that sleep disturbance was a significant contributor to increased suicidal ideation in depressed postpartum women.
