Introduction

Image-guided surgery (IGS) is a real-time correlation of the operative field to preoperative imaging, showing the precise location of a selected surgical instrument within the surrounding structures. The application of image guidance in endoscopic sinus surgery has revolutionized the field of rhinology by providing the surgeon with optimal anatomic localization, improving patient safety, minimizing invasiveness of difficult chronic sinus and extirpative procedures, and is at the forefront in the transition to minimally invasive sinus surgery for the management of chronic sinusitis, skull base lesions, repair of CSF leaks, and orbital surgery. Although instrumental in the evolution of the field of rhinology, IGS does not replace proper training and knowledge of anatomy.
