Zusammenfassung
Die Analfissur ist ein häufiges proktologisches Krankheitsbild und bedingt meist einen hohen Leidensdruck. Die chronische Analfissur sollte in der Regel zunächst einen konservativen Therapieversuch erfahren. Bleibt der Therapieerfolg aus, besteht eine Operationsindikation. Bei einer Fistel oder sehr starken Schmerzen kann auch primär die Operation empfohlen werden, bei Nachweis eines Abszesses sollte ohnehin ein chirurgischer Eingriff erfolgen. Im Jahr 2020 wurde eine aktuelle Version der deutschen Leitlinie „Analfissur“ publiziert. Die Operationsmethode der ersten Wahl ist die Fissurektomie mit Exzision aller Sekundärveränderungen unter Anlage eines ausreichend großen Drainagedreiecks. Der Sphincter ani internus wird dabei freigelegt. Der Advancement-Flap und die Injektion von Botulinomtoxin sind untergeordnete Optionen, die laterale Internussphinkterotomie sollte nur in absoluten Ausnahmen erfolgen. Im Rahmen der postoperativen Behandlung erscheint der frühzeitige Beginn einer Dehnungsbehandlung vorteilhaft.
Abstract
Anal fissure is a common proctologic pathology and is usually associated with severe suffering. Chronic anal fissure are initially treated using conservative options. If this fails, surgery is indicated. If a fistula is detected, if the level of pain is too intense, or if there is any evidence for an abscess, surgery could be primarily recommended. An updated version of the German guideline “Anal fissure” was published in 2020. The recommended surgical technique is fissurectomy including the excision of all secondary pathologies. The surface of the internal sphincter is laid open, and the perianal triangle for secondary healing should be sufficiently large. The advancement flap and the injection of botulinum toxin are secondary options; a lateral internal sphincterotomy should only be rarely performed. In the context of postoperative care, early anal dilatation throughout postoperative treatment is advantageous.
