Zusammenfassung
Das TraumaRegister DGU® wurde 1993 als Initiative von 5 Kliniken gestartet und seitdem kontinuierlich weiterentwickelt. Es werden dort definierte Daten zu vier Zeitpunkten der Versorgung schwerverletzter Patienten, vom Unfallort bis zur Entlassung aus der Klinik, erfasst. Im ersten Dokumentationsjahr (1993) waren 260 Patienten aus 5 Kliniken involviert, im letzten Dokumentationsjahr (2015) wurden mehr als 38.000 Patienten aus 640 Kliniken erfasst.
Ziel der vorliegenden Arbeit ist die Bewertung des Einflusses des TraumaRegister DGU® auf die Versorgung Schwerverletzter in Deutschland. Verschiedene Autoren konnten zeigen, dass die Datennutzung im Rahmen der externen Qualitätssicherung einer Klinik helfen kann, Versorgungsprobleme zu erkennen, Lösungen zu erarbeiten, und daraus abgeleitete Maßnahmen neu zu bewerten. Es wurden strukturelle und prozessuale Veränderungen vorgenommen. Infolgedessen konnte der Zeitbedarf für die Diagnostik von Schwerverletzten deutlich reduziert werden. Verschiedene wissenschaftliche Auswertungen der Daten des TraumaRegister DGU® haben Eingang in die Versorgung Schwerverletzter gefunden. Während der präklinischen Versorgung wurde die Indikation zur Intubation bei atemsuffizienten Schwerverletzten kritischer gestellt, und das Volumen deutlich reduziert. In der klinischen Versorgung hatte die Datenauswertung einen erheblichen Einfluss auf die Bildgebung und auf die Therapie von Gerinnungsstörungen. Ferner konnte die risikoangepasste Mortalität Schwerverletzter in Deutschland in den vergangenen 20 Jahren kontinuierlich gesenkt werden.
Abstract
The German Trauma Registry DGU® started in 1993 as an initiative of five dedicated trauma centers and has evolved significantly since then. Data were obtained at four points of time from the site of the accident until discharge from hospital. In the first year (1993), the registry collected data of 260 patients from 5 hospitals. In 2015 more than 38.000 were included from 640 hospitals.
This paper focusses on the impact of the trauma registry on the treatment of severely injured patients. Several authors could show that the data can be used by hospitals for benchmarking. This can help to detect problems in individual hospitals and to find solutions that can be implemented into the process of care and its subsequent reevaluation. Due to structural and process-related changes, the time necessary for the management in the emergency room could be reduced significantly. Various scientific analyses of the Trauma Registry DGU® data were implemented in the treatment of severely injured patients. In the prehospital treatment, this changed the criteria for intubation and led to a reduction of volume replacement. In the hospital setting, the analysis influenced the radiologic work-up and the treatment of coagulopathy of severely injured patients. Moreover, the risk-adjusted mortality of severely injured patients in Germany could be continuously reduced over the past 20 years.
