Alzheimer-Demenz oder zerebrale Toxoplasmose? Falldarstellung einer Patientin mit Demenz nach Toxoplasmose-Infektion

Zusammenfassung

Die zerebrale Toxoplasmose kann bei AIDS und bei Immunschwäche zu Demenz führen. Wir möchten einen Fall vorstellen, in dem eine zerebrale Toxoplasmose mit einer Alzheimer-Demenz (AD) assoziiert war. Die Patientin hatte ein halbes bis ein Jahr vor Beginn der ersten kognitiven Beeinträchtigungen mit 56 Jahren eine subakute Infektion mit Toxoplasmose durchgemacht. Sowohl die neuropsychologische Abklärung als auch das MRI erweckten initial den Verdacht eines demenziellen Prozesses entzündlicher Genese. Die Laboruntersuchungen einschließlich der Liquoruntersuchung erbrachten jedoch nur wenig entzündungsspezifische Resultate. Das PET zeigte Veränderungen des Glukosestoffwechsels, die für eine AD eher typisch sind, sodass aus Sicht der Autoren von einer Komorbidität einer zerebralen Toxoplasmose mit einer AD auszugehen ist.

Summary

Cerebral toxoplasmosis can lead to dementia in AIDS and in immunodeficient patients. We present a case study in which cerebral toxoplasmosis was associated with a dementia of Alzheimer type. Half a year to one year before the cognitive impairment began, the patient suffered a subacute infection of toxoplasmosis at the age of 56. Neuropsychological examination as well as MRI suggested a diagnosis of dementia with infectious genesis. However, serological tests showed only little evidence of infection. Since the results of the PET examination indicated changes in the glucose metabolism typical of Alzheimer's disease, we infer a comorbidity of cerebral toxoplasmosis and dementia of Alzheimer type.
