Abstract
Arterial skeletonization before radioembolization or hepatic artery port catheter placement for chemotherapy is a crucial step to prevent side effects. Coils are commonly used as an embolic material for hepatoenteric arterial occlusion. Herein is reported for the first time the clinical use of a new detachable microvascular plug (MVP) suitable for occlusion of 1–3 mm diameter vessels. The MVP allows vessel embolization even in challenging anatomy such as the right gastric artery. Furthermore, immediate and stable vessel occlusion was observed in our pilot study of 16 MVP placements.

A 45-year-old woman with liver-dominant colorectal cancer metastases was referred to intra-arterial hepatic port catheter placement for further chemotherapy. Through a 4F sheath, the right gastric artery was catheterized with a 2.7F microcatheter coaxially mounted to a 4F shepherd catheter in the celiac trunk. The MVP was then deployed into the target artery. As a result of inadequate position, the MVP was resheathed and redeployed in a better position. The MVP was then detached by electrolytic release. Follow-up angiography performed through the port catheter revealed permanent occlusion of the target artery. Supplementary material 1 (MP4 1623 kb)
