Zusammenfassung
Aus der Sicht einer funktionellen Psychopathologie wird postuliert, dass bei Jugendlichen über die strenge Nosologieorientierung hinaus ein diagnostischer Blick auf alterstypische Lebensthemen und Entwicklungsaufgaben gerichtet werden muss, wie ihn nur eine entwicklungspsychopathologische Sichtweise erlaubt. Die nosologische Zuordnung der Symptome muss durch eine strukturelle Analyse der Ressourcen des Selbst und eine funktionelle Analyse des Kontextes ergänzt werden. Die Symptome besitzen immer auch eine Anpassungsfunktion und sind niemals zufällig in ihre psychosozialen Kontexte eingebettet. Kybernetische Modelle erlauben eine Beschreibung des zielgerichteten Verhaltens. Im Regelkreismodell dient das Verhalten dazu, die eigene Wahrnehmung der Umweltbedingungen so zu beeinflussen, dass diese einem inneren Richtwert – dem Ziel – angenähert werden. In Beispielen wird illustriert, wie klinische Symptome in funktioneller Weise mit den Lebensumständen verquickt sein können und aus dieser Funktionalität heraus sich einer rein symptomorientierten Therapie verschließen. Die Beachtung funktioneller Verflechtungen der Symptome dient demgegenüber einem individualisierten Vorgehen in der Therapie mit Jugendlichen.
Abstract
From the perspective of functional psychopathology it is postulated that for adolescents a diagnostic view must be directed towards age-related topics of life and developmental tasks beyond the strict nosological orientation, which can only be achieved from a developmental psychopathological perspective. The nosological assignment of symptoms must be complemented by a structural analysis of the resources of the self and a functional analysis of the context. The symptoms always also have an adaptation function and are never accidentally embedded in the psychosocial context. Cybernetic models allow a description of the targeted behavior. In the closed loop system behavior serves to influence the self-perception of environmental conditions in such a way that they become approximated to an internal reference point: the target. Examples are given to illustrate how clinical symptoms can be intertwined with the personal circumstances in a functional way and from this functionality a purely symptom-oriented therapy can be derived. In contrast, consideration of the functional interweavement of the symptoms serves as an individualized approach in the therapy with adolescents.
