Zusammenfassung
Fragestellung
Vergleich der Häufigkeit spontaner Blutungsereignisse im Bereich des Auges unter der oralen Antikoagulation mit Apixaban und Phenprocoumon.
Methode
In dieser retrospektiven Datenerhebung wurden Patienten, die sich zwischen dem 01.01.2015 und dem 31.06.2015 unter einer dauerhaften Antikoagulation mit Phenprocoumon oder Apixaban befanden und an der Universitätsaugenklinik Münster vorstellten, eingeschlossen. Ausgewertet wurden symptomatische, spontan auftretende Blutungen im Bereich der Netzhaut und des Glaskörpers. Erfasst wurden folgende Parameter: ophthalmologische Diagnose, Alter, Visus, Begleitmedikation und Begleiterkrankungen. Der Zusammenhang zwischen Behandlungsgruppe und Blutung wurde mit dem exakten Test nach Fisher getestet.
Ergebnisse
Insgesamt wurden 172 Patienten (Alter = 74,0 ± 10,6; 57,0 [n = 98] männlich; 43,0 % [n = 74] weiblich) in die Studie eingeschlossen. 147 Patienten erhielten eine Therapie mit Phenprocoumon, hiervon erlitten 5 Patienten (3,4 %) ein spontanes Blutungsereignis der Netzhaut oder des Glaskörpers. In der Apixaban-Gruppe (25 Patienten) kam es bei 9 Patienten (36 %) zu einem dieser spontanen Blutungsereignisse. Der Zusammenhang zwischen Behandlungsgruppe und Blutungshäufigkeit war signifikant (p < 0,001).
Diskussion
In dem untersuchten Patientenkollektiv ist die Blutungshäufigkeit in beiden Vergleichsgruppen signifikant unterschiedlich. Eine gewisse Beschränkung der Studie ist durch die geringe Anzahl von Patienten, durch das retrospektive Studiendesign und durch die fehlende Randomisierung und Verblindung gegeben. Weitere Studien, insbesondere bei Patienten mit einem Risiko für Spontanblutungen der Netzhaut oder des Glaskörpers (Altersabhängige Makuladegeneration [AMD]), proliferative diabetische Retinopathie) sind dringend notwendig.
Abstract
Background
To determine and compare the frequency of intraocular hemorrhage in patients who underwent oral anticoagulation with apixaban or phenprocoumon.
Methods
A retrospective analysis of patients under oral anticoagulant medication (apixaban or phenprocoumon) seen between January 2015 and June 2015 at the department of ophthalmology, University of Muenster Medical Center was performed. Vitreal or retinal hemorrhage in addition to clinical information including age, gender, best corrected visual acuity, concomitant diseases, concomitant medication and therapy were obtained. Bleeding frequency in both groups was compared using Fisher’s exact test.
Results
A total of 172 patients were included with a mean age = 74.0 ± 10.6 years, 57.0% (n = 98) male and 43.0% (n = 74) female. In the phenprocoumon group 147 patients (3.4%, n = 5) developed a retinal or vitreal hemorrhage. In the apxiban group 25 patients (36%, n = 9) developed a retinal or vitreal hemorrhage. There was a significant correlation between the group and bleeding risk (p < 0.001).
Conclusion
There was a significant correlation between medication (apixaban vs. phenprocoumon) and bleeding risk in this study population. Further studies with more patients especially in patients with a high risk of hemorhage, age-related macular degeneration (AMD) and proliferative diabetic retinopathy are needed.
