Zusammenfassung
Amyloidosen sind Erkrankungen, die durch fehlgefaltete Proteine verursacht werden. Die Fehlfaltung bewirkt, dass die hierbei entstehenden fibrillären Proteine unlöslich werden, sich in Geweben und Organen ablagern und dort zu schweren Fehlfunktionen führen können. Vor allem im Rahmen von Malignomen, chronischen Infektions- oder Systemerkrankungen, aber auch stets, wenn eine anderweitig nicht erklärbare Nephropathie, Kardiomyopathie, Neuropathie, Enteropathie, Arthropathie oder eine Makroglossie mit oder ohne periorbitale Einblutungen vorliegt, sollte eine Amyloidose in Erwägung gezogen werden. Die Diagnose kann in der Regel durch eine Probe aus dem Bauchfett, aus dem Rektum oder aus betroffenen Organen gestellt werden. Die Therapie zielt vor allem auf eine Reduktion der Aktivität der Grunderkrankung und der spezifischen Organprotektion. Neuere Therapiestrategien umfassen die Hemmung von Interleukin-1, aber auch Hemmstoffe der Proteinfaltung selbst.
Abstract
Amyloidoses make up a group of diseases caused by misfolded proteins. These misfolded proteins are insoluble and are deposited in various tissues and organs, ultimately resulting in severe organ dysfunction. The majority of patients with amlyoidoses suffer from chronic inflammatory, infectious or malignant diseases. Moreover, unexplained nephropathy, cardiomyopathy, neuropathy, enteropathy, arthropathy or macroglossia with or without periorbital bleeding should include an amyloidosis in the differential diagnosis. The latter is facilitated by histological examination of abdominal adipose tissue, the rectum or affected organs. Therapy focuses predominantly on reduction of activity of the underlying disease and specific organ protection. More recent therapeutic strategies include interleukin-1 inhibition, as well as inhibitors of protein misfolding.
