Zusammenfassung
Hintergrund
Der Einsatz von Telemedizin im Rettungsdienst wird in Deutschland seit einigen Jahren erprobt und eingesetzt. Dies ist eine bedeutsame Entwicklung zur Aufrechterhaltung einer flächendeckenden medizinischen Versorgung in abgelegenen Gebieten sowie bei stetig steigenden Einsatzzahlen im prähospitalen Sektor.
Ziel der Arbeit
Die technische Zuverlässigkeit des hier gewählten Systems soll im ländlichen Bereich mit teils schlechter Netzwerkabdeckung erprobt werden.
Material und Methoden
Am 04.01.2021 wurde im Rettungsdienst des Landkreises Goslar im Rahmen des Pilotprojekts „Telenotfallmedizin Niedersachsen“ ein telenotfallmedizinischer Arbeitsplatz eingerichtet. Die technische Zuverlässigkeit des Systems sowie Einsatzindikationen und Auslastung wurden untersucht. Sämtliche Einsätze des Telenotarztes (TNA) zwischen dem 04.01.2021 und dem 31.12.2021 wurden analysiert.
Ergebnisse
Es fanden 1968 telenotfallmedizinische Einsätze statt. 97 % der Einsätze wurden vom TNA eigenständig abgearbeitet. In 1,1 % der Fälle musste der Einsatz aufgrund technischer Schwierigkeiten abgebrochen werden, in 1,9 % musste aus medizinischen Gründen ein regulärer Notarzt nachgefordert werden.
Diskussion
Das genutzte System ist technisch zuverlässig. Notfalleinsätze konnten von einem Telenotarzt größtenteils eigenständig abgearbeitet werden.

Background
Telemedicine in prehospital emergency services has been tested and implemented in Germany during the past years. This is an important development to ensure widespread provision of medical healthcare even in rural areas and to meet rising numbers of ambulance transports.
Objective
To examine the technical reliability of the chosen telemedical system in a rural area with partially poor network reception.
Methods
Since 04 January 2021, a workplace for a tele-emergency physician (TEP) has been set up in the district of Goslar as part of a pilot project. Reliability of the system, indications for deployment and utilization were examined. All missions of the TEP between 04 January 2021 and 31 December 2021 were analyzed. Technical problems during transmission and the need to request a regular emergency physician were also analyzed.
Results
There were 1968 missions of the TEP, whereby 97% of missions were completed by the TEP alone and 1.1% of missions were aborted due to technical problems. In 1.9% of cases, a regular emergency physician was needed due to medical reasons.
Conclusion
The system used was technically reliable. Emergency missions were managed for the most part by a TEP alone.
