Résumé
Le risque d’exposition et d’intoxication par les agents chimiques de guerre ne peut être exclu comme l’illustrent malheureusement les événements récents en Syrie. Utilisés lors de conflits (guerre Iran-Irak) ou lors d’attentats au Japon (1994–1995), les neurotoxiques organophosphorés (NOP) sont des agents létaux agissant en inhibant les cholinestérases présentes dans les tissus et organes, dont l’acétylcholinestérase, enzyme essentielle au fonctionnement des systèmes nerveux central et périphérique (système nerveux autonome et moteur). L’expérience acquise lors d’événements accidentels ou agressifs / malveillants et par l’expérimentation animale a permis de définir les modalités de prise en charge médicale des intoxications aiguës aux NOP. Après une présentation générale sur les NOP et la symptomatologie de l’intoxication aiguë, les principes de cette prise en charge médicale seront rappelés.
Abstract
Chemical warfare agents represent an ever-lasting threat, not only for military forces but also for civilian populations as illustrated by the recent events in Syria. Nerve agents such as those used by Iraq during the Iran/Iraq war or by the Aum Shinri Kyo sect in Japan are among the deadliest of those non-conventional weapons. Their effects are mainly due to their ability to inhibit cholinesterases throughout the body. After a short review of the main characteristics of nerve agents and of the pathophysiology and clinical presentation of acute nerve agent poisoning, the general principles of clinical management will be reviewed.
