Abstract

As clinical and instrumental capabilities become more sophisticated, the number of recognized mechanisms by which cerebral ischemia may occur has grown. In recent years, it has been recognized that cerebral ischemia may be due to occlusive disease of large or small intracranial arteries or large extracranial arteries, cardioembolic (including transcardiac) or intra-arterial embolism, inflammatory diseases, arterial anomalies etc. In some patients hemorrhagic cerebrovascular disease may be complicated by ischemia as well. Increasing attention has refocused on the old concept that cerebral vasoconstriction or ’vasospasm’ may be an important cause of cerebral ischemia. Basic neuroanatomical and neuropharmacological investigations have defined the physiological aspects of the aminergic and peptidergic pathways and mechanisms involving the cerebral circulation. In addition, a number of reports discuss the exploding field of endothelium-derived factors which may affect cerebral vascular tone.
