Abstract

Communication failure has been a well-identified cause of medical error and subsequent patient injury. Despite numerous studies of how poor communication impacts team performance and articles suggesting strategies for improving communication, preventable medical errors persist as the number 3 killer in the United States. The Joint Commission has cited communication as the number one root cause of sentinel events. There are different types of communication failure and many factors that contribute to it. Challenges to effective communication can be mitigated by participating in formalized communication improvement processes and engagement in both personal and group training. Standardization of methods and utilization of structured communication tools can support high-level communication, personalization of patient care, and decreased errors.
