Abstract
Along the years, scientific clinical data have been collected concerning the possible saphenous flow restoration without any ablation and according to the CHIVA strategy (Gianesini et al. Phlebology 30:157–71, 2015). Since 1988, when C. Franceschi gave the first description of this strategy (Franceschi 1988), the scientific debate has been opened regarding the success of the saphenous-sparing therapeutic approach to treat chronic venous disease (Fichelle et al., J Mal Vasc. 17:224–228, 1992; Franceschi J Mal Vasc. 17(4):291–300, 1992; Bahnini et al. 1997). A first series of studies have been published during the twentieth century, addressed to compare these minimally invasive techniques with the current surgical practice on varicose veins; however, the series included few prospective case-control studies (Franceschi et al. Int Angiol. 35(1):8–30, 2016). Subsequently, some randomized controlled trials were published in order to give more evidence on the use of CHIVA (Carandina et al. Eur J Vasc Endovasc Surg. 35:230–237, 2008; Iborra et al. ANGIOLOGÍA 58(6):459–468, 2006; Parés et al. Ann Surg. 251:624–631, 2010). Two Cochrane Reviews highlighted the smaller recurrence risk following a CHIVA strategy rather than a saphenous stripping (Bellmunt-Montoya et al. Cochrane Database Syst Rev. CD009648, 2013 and Cochrane Database Syst Rev. CD009648, 2015). The aim of this chapter is to summarize the scientific literature on CHIVA technique.
