Zusammenfassung

Die untere Mundhöhlenetage umfasst die Strukturen zwischen Unterkieferbasis und Kauebene und schließt die Mandibula, den Mundboden und die orale Zunge einschließlich der vaskulären und nervalen Leitungsstrukturen ein. Resektionen in diesem Bereich betreffen die wesentlichen Funktionen von Sprache und Kommunikation, Schluckakt, Mastikation und Respiration. Der Unterkiefer besitzt zudem eine anatomische Schlüsselfunktion, welche die Integrität und Ästhetik des unteren Gesichtsdrittels entscheidend beeinflusst. Nicht ohne Grund ist der Verlust des Kinnbogens als »Andy Gump-deformity« in der Literatur bekannt. Dem Anspruch, eine radikale Tumorresektion bei möglichst weitgehendem Erhalt von Funktion und Ästhetik zu gewährleisten, muss in der Abstimmung von Resektionstechnik, chirurgischem Zugang und geeigneten rekonstruktiven Verfahren Rechnung getragen werden. Die Auswahl des Resektions- und Rekonstruktionsverfahrens muss auch potenzielle adjuvante Therapiemodalitäten (Radiotherapie, [Radio-]Chemotherapie) berücksichtigen.
