Zusammenfassung

Nachdem anfänglich die Harnableitung und der Schutz des oberen Harntraktes im Vordergrund standen, ist die Entwicklung der letzten Jahre in Richtung einer möglichst funktionellen, und wenn möglich anatomischen, Rekonstruktion gelaufen. Nach der inkontinenten kutanen Harnableitung mittels Conduit wurde die kontinente kutane Harnableitung mittels katheterisierbarem Pouch und daraus die kontinente orthotope Harnableitung weiterentwickelt, die sich mit guten funktionellen Resultaten bei beiden Geschlechtern durchgesetzt hat.

Die Geschichte des orthotopen Blasenersatzes ist noch relativ jung; er wird weltweit immer häufiger angewendet. Vergleichende Studien gibt es jedoch noch kaum.

In den letzten Jahren hat die Zahl der inkontinenten Harnableitungen wieder etwas zugenommen, da aufgrund der verbesserten Anästhesie- und Operationstechnik vermehrt ältere Patienten operiert und mit Conduits versorgt werden.

Zur Anwendung der verschiedenen Harnableitungen gibt es die Empfehlungen der WHO-Consensus Conference on Bladder Cancer.
