Zusammenfassung
Kapitel 4 der Internationalen Klassifikation von Kopfschmerzerkrankungen beinhaltet eine klinisch sehr heterogene Gruppe primärer Kopfschmerzen. Über die Pathogenese dieser Kopfschmerztypen ist nur wenig bekannt. Die Therapie basiert in der Regel auf Einzelfallberichten und unkontrollierten Studien. Zu den beschriebenen Formen gehören der primäre stechende Kopfschmerz, der primäre Hustenkopfschmerz, der primäre Kopfschmerz bei körperlicher Anstrengung, der primäre Kopfschmerz bei sexueller Aktivität, der primäre schlafgebundene Kopfschmerz, der primäre Donnerschlagkopfschmerz, die Hemicrania continua und der neu aufgetretene Dauerkopfschmerz. Einige der aufgeführten Kopfschmerztypen können symptomatischer Natur sein und erfordern eine sorgfältige Untersuchung mithilfe von bildgebenden und anderen Verfahren. Primäre und sekundäre Kopfschmerzen müssen dabei sorgfältig abgegrenzt werden.
