Direct Effects of Diuretic Drugs on the Myocardium

Abstract

Recently extrarenal, mainly cardiac effects of kaliuretic and especially antikalioretic diuretics have been repeatedly reported (1, 11, 12, 17, 18). Experimental results indicate that aldosterone antagonists, triamterene and amiloride, effectively prevent the occurrence of cardiac arrhythmias and may reduce glycoside sensitivity of the heart not only by their antikaliuretic effects but also by a possible direct action on the myocardium (4, 7, 15, 19, 21, 23). Its mechanism is not yet sufficiently clarified. With regard to the broad application of these substances it was the aim of this study to prove whether kaliuretic and antikaliuretic diuretics may influence myocardial membrane properties. Electrophysiologic parameters of myocardial excitability were studied by the microelectrode technique under the influence of furosemide, thiabutazide, potassium canrenoate, amiloride and triamterene.
