Zusammenfassung
Das Reninsystem bei Patienten mit Diabetes mellitus stößt in letzter Zeit wieder auf ein vermehrtes Interesse. Dies liegt darin begründet, daß pharmakologische Hemmsubstanzen des Reninsystems mit Erfolg unter verschiedenen Indikationen bei Patienten mit Diabetes mellitus eingesetzt werden. Ziel der vorliegenden Arbeit ist es, eine Übersicht über die Literatur zum Reninsystem bei Diabetikern zu geben. Diese Übersicht zeigt in Übereinstimmung mit eigenen Befunden, daß die Aktivität des Renin-Angiotensin-Systems (RAS) bei Diabetikern mit ausreichender Stoffwechselkontrolle normal oder eher gesteigert ist. Bei Patienten mit einer diabetischen Nephropathie wird eine erhöhte, aber auch eine verringerte Aktivität des RAS angegeben. Die Befunde widersprechen der häufig vorgetragenen Vermutung, daß das RAS bei Diabetes mellitus generell supprimiert und funktionell inaktiv sei. Gegen diese Vermutung sprechen Befunde beim Menschen, die einen eher erhöhten Angiotensin-II-Rezeptorbestand zeigen. Auch die Pressorantwort auf infundiertes Angiotensin II ist beim Diabetiker deutlich gesteigert. Diese gesteigerte Ansprechbarkeit ist um so bedeutsamer, da beim Diabetiker das RAS nicht durch den erhöhten Körpernatriumbestand supprimiert ist. Eine Reihe von Daten sprechen dafür, daß auch die Widerstandsgefäße der Niere beim Diabetiker vermehrt auf Angiotensin II reagieren. Hier könnte eine Ursache für die bekannte Hyperfiltration liegen. Jedenfalls wird übereinstimmend von verschiedenen Autoren eine druckunabhängige Verminderung der Mikroalbuminurie nach Hemmung des RAS — vor allem bei Typ I Diabetes — gefunden. Dies ist ein möglicher indirekter Hinweis auf eine Verminderung des glomerulären kapillaren Drucks, wobei Veränderungen der Siebeigenschaft der Basalmembran, tubuläre Rückresorption von Albumin sowie die glomeruläre Oberfläche nicht unberücksichtigt bleiben dürfen.
