Das stumpfe Knorpeltrauma — Modell für die Entstehung einer Chondropathie

Zusammenfassung

Für das klinische Bild eines patellofemoralen Schmerzsyndroms ist überwiegend eine Chondropathie der retropatellaren Gelenkfläche verantwortlich. Hauptursache dieser Chondropathie sind Makro- und Mikrotraumen, wie sie besonders häufig im Sport auftreten.

Wir haben im Tierexperiment ein stumpfes Knorpeltrauma erzeugt und die Sequenz morphologischer Veränderungen im Elektronenmikroskop verfolgt.

In der Druckzone des Knorpels kommt es zur Nekrose mit konsekutiver Änderung der Elastizität des Knorpels. Daraus resultiert eine Überlastung des subchondralen Knochens. Das schmerzfreie Intervall zwischen Trauma und wiederauftretendem Schmerz wird erklärt, die mögliche Schmerzursache dieses Syndroms diskutiert.

Summary

The well recognized clinical syndrom of patellofemoral pain often is caused by lesions of the articular cartilage (chondromalacia patellae). Chondromalacia patellae is mainly the result of micro- and/or macrotrauma especially in athlets.

Experiments with rabbits were performed and the morphological changes following experimental trauma were examined under the electron microscope (TEM).

In the pressure zone of the cartilage degeneration and necrosis occur leading to a decrease in the elasticity of the cartilage and an alteration in the distribution of pressure on the subchondral bone.

The painfree interval between trauma and the recurring pain as well as the possible cause of pain will be discussed.
