Zusammenfassung

Der Begriff Chemonukleolyse bezeichnet die nichtoperative, enzymatische Beseitigung von Nucleus-pulposus-Material. Lyman Smith (1969) prägte diesen Begriff im Zusammenhang mit der intradiskalen Injektion von Chymopapain bei lumbalen Bandscheibenvorfllen. Nach der offiziellen Nomenklatur der American Academy of Orthopedic Surgeons wird dieser Begriff seit 1980 generell für die intradiskale Injektion von Enzymen bei lumbalen Bandscheibenvorfällen vorgeschlagen. Erste Erfahrungen wurden mit der Protease Chymopapain gesammelt, die bereits 1963 klinische Anwendung fand. Mittlerweile sind mit diesem Medikament, einem Extrakt der Papaya-Frucht, etwa 70 000 Patienten behandelt worden.
