Zusammenfassung
Ventrikuläre Spätpotentiale sind elektrische Signale mit einer Amplitude an der Körperoberfläche von wenigen tausendstein Millivolt, die durch eine verzögerte Erregung umschriebener Myokardareale entstehen (1, 2). Mit invasiven Verfahren sind sie vom Endokard und Epikard als fragmentierte Potentiale von wesentlich höherer Amplitude (0,1–1 mV) ableitbar (3, 4). Es konnte gezeigt werden, daß der Nachweis von Spätpotentialen während Sinusrhythmus auf einer pathologischen Leitungsverzögerung in organisch geschädigtem Myokard beruht; sie lassen somit auf strukturelle Veränderungen am Myokard schließen, welche die Voraussetzung für kreisende Erregung sind (2). In den meisten Fällen liegt eine koronare Herzkrankheit mit abgelaufenem Herzinfarkt vor, Spätpotentiale ließen sich jedoch auch bei Patienten mit Kardiomypathie (5), QT-Syndrom (6) und arrhythmogener rechts ventrikulärer Dysplasie (1) nachweisen.
