Abstract
Surgical decompression in malignant endocrine exophthalmos (Graves’ disease) is indicated when the patient’s health does not improve through regulation of thyroid metabolism and/or systemic corticoid therapy. A compression optic neuropathy, motility disorder, lagophthalmos, and cosmetic impairment found in these cases support the indication for a surgical approach. The rhinosurgical solution requires preparing the paranasal system either partially [3, 8, 9] or completely [1, 6, 7] as needed for the uptake of the orbital fatty tissue. Besides technical recommendations for the orbital decompression procedure, developed by Richter and Buschmann [1, 2, 6], the results of 83 decompressions are presented; these were performed from 1984 to 1988 in close cooperation with the Ophthalmic Department of the University of Würzburg.
