Zusammenfassung

Die Myokarditis ist definiert als ein sich herdförmig oder diffus im Herzmuskel ausbreitender, akut oder chronisch-rezidivierend verlaufender Entzündungsprozeß, der durch eine direkte oder indirekte Einwirkung von Erregern, von toxischen, chemischen oder physikalischen Agenzien oder durch bisher unbekannte Ursachen hervorgerufen wird. Die Mehrzahl der Myokarditiden verläuft klinisch inapparent. Dies gilt besonders für virusinduzierte Myokarditiden, die, nachdem die bakteriell verursachten Myokarditiden aufgrund einer gezielten antibiotischen Therapie deutlich seltener beobachtet wurden, den Hauptanteil aller Myokarditiden ausmachen. Obwohl zahlreiche kardiotrope Viren prinzipiell als Erreger in Betracht kommen, stehen aufgrund epidemiologischer Untersuchungen Enteroviren (RNA-Viren), und hier speziell die Coxsackie-Viren der Gruppe B (CBV), als auslösendes Agens an erster Stelle (Abb. 1.4.1).
