Zusammenfassung

Die Minuten und Stunden nach der Geburt des Kindes sind für die Mutter die gefährlichsten der ganzen Schwangerschaft. Die Pathologie der Plazentarperiode, die vor allem geprägt ist durch erheblichen Blutverlust, ist so gravierend, daß sie meist nur durch raschen und massiven Einsatz aller technischen und labormedizinischen Mittel und durch das reibungslose Zusammenspiel unterschiedlicher Fachdisziplinen bewältigt werden kann. Hauptursache für postpartale Blutungen sind die Uterusatonie mit und ohne Plazentaretention sowie Rißverletzungen im Bereich der Geburtswege.

Auf starken Blutverlust pfropft sich geradezu gesetzmäßig eine Gerinnungsstörung auf, die nur mit dem Einsatz von Blutprodukten und Gerinnungsfaktoren beherrscht werden kann. Eine wesentliche Rolle kommt der Prävention zu: Die aktive Leitung der Nachgeburtsperiode mit Oxytozin, möglichst rasch nach dem Durchtritt der kindlichen Schultern, führt zu wesentlich geringerem Blutverlust als die abwartende Haltung. Genaue Anamnese, Risikoeinschätzung und rechtzeitige Vorbereitung auf den möglichen Notfall bei gefährdeten Patientinnen ersparen Überraschungen. Ebenso gehört die genaue Beobachtung der Wöchnerin in den Stunden nach der Geburt zu den Pflichten eines geburtshilflichen Teams, sowie die sachgerechte und griffbereite Lagerung der Uterotonika Oxytozin, Prostagiandin und Methylergometrin, damit sie im Bedarfsfall rasch zubereitet und verabreicht werden können.
