Dens invaginatus en dens evaginatus: profylaxe en behandeling

Inleiding

Een dens invaginatus is een abnormaliteit in de ontwikkeling waarbij het glazuurorgaan gedesorganiseerd is en leidt tot een invaginatie in het oppervlak van de tandkroon vóór het optreden van calcificatie. Deze invaginatie is begrensd door glazuur. Ook andere namen zoals dens in dente worden gebruikt. Dens invaginatus lijkt echter de meest geschikte naam, omdat hierbij het inplooien van de buitenzijde (glazuur) in de binnenzijde (dentine) met de vorming van een blinde ruimte of pocket wordt weergegeven. Dens invaginatus is een niet ongewone klinische vondst waar gemakkelijk overheen wordt gekeken, omdat significant klinische tekenen van de anomalie in veel gevallen afwezig zijn. Het kan klinisch worden gediagnosticeerd als een tand met ongewone kroonmorfologie of met een diep foramen caecum . Radiografisch ziet men het invouwen van een radiopake lint- of peervormige structuur met een densiteit gelijk aan glazuur. De aanwezigheid van een invaginatie wordt gezien als een risicofactor voor het ontstaan van cariës, pulpale pathologie of parodontale inflammatie. Vroege opsporing is dus belangrijk.
