Liver resection for colorectal metastases: who, when, and to what extent?

Abstract

Progress in the understanding of the natural history of liver metastases from colorectal cancer as well as improvements in the techniques and technology have permitted the development of curative hepatic surgery over the past two decades. With the accumulation of a large amount of retrospective data from numerous institutions, the time has now come when critical questions must be investigated such as: 1) who are candidates for liver resection, 2) when should resection be performed after the diagnosis of liver metastasis, 3) what is the extent of liver resection necessary for optimal survival rates, and 4) what criteria preclude hepatic resection? These questions will best be answered prospectively, but by analysis of personal experience and review of the available data we hope to provide an initial framework with which to begin to analyze the problems.
