Zusammenfassung

Hepatitis E ist eine infektiöse Leberentzündung, die durch das Hepatitis-E-Virus (HEV) ausgelöst wird, zunehmend auch autochthon in Industrienationen. Es lassen sich fünf HEV-Genotypen unterscheiden, die sich u. a. bzgl. ihrer geographischen Verbreitung unterscheiden. Der Verlauf einer HEV-Infektion reicht von klinisch asymptomatisch bis zur Entwicklung eines akuten Leberversagens. Bei der akuten Hepatitis E folgen auf ein Prodromalstadium mit unspezifischen grippalen Symptomen sowie gastrointestinalen Symptomen meist Hepatitis-spezifischere Beschwerden (wie Ikterus, Dunkelfärbung des Urins und zunehmende Entfärbung des Stuhls). Chronische Verläufe wurden in Deutschland bisher nur bei Immunsupprimierten gefunden. Der sensitivste Test auf eine akute Hepatitis E ist der Nachweis des Virus im Stuhl per PCR, alternativ im Blut. Ribavirin und Interferon werden in der Behandlung der akuten und der chronischen Hepatitis E eingesetzt.
