Abstract
The authors propose a clinico-radiologic decision tree approach to solve the problem of a chid with acute abdominal. This approach is based on clinical conditions, age and gamuts of differential diagnosis. The couple US + plain film of the abdomen should solve most of the cases. Ultrasound examinations should be optimized using high resolution transducers with settings, fitted to the child size and weight. The entire abdominal cavity should be examined. In selected cases, CT (dosis fitted to children) and MR imaging provide useful information. Most frequent diagnosis include appendicitis and its mimickers. Once appendicitis and other intestinal diseases are excluded, extra digestive (i.e. gynaecologic diseases) conditions and extra abdominal conditions should be considered (i.e. pneumonia).
