JR is a 34-year-old man who presented with complaints of epiphora on the left side for several months. He had been given antibiotics and decongestant nasal sprays by his primary care physician without relief of the tearing. He was referred to an ophthalmologist, who noted some fullness in the nasolacrimal fossa and found a firm lesion on palpation.
