Abstract
Nonneoplastic and neoplastic lesions of the lymph node and spleen are described, including morphology, immunophenotype, genetics, and differential diagnoses. Categories of nonneoplastic lesions include follicular hyperplasia, paracortical hyperplasia, sinus hyperplasia, and necrotizing granulomatous and nongranulomatous lymphadenitis. Categories of neoplastic lesions include B-cell neoplasms, T-cell neoplasms, NK-cell neoplasms, Hodgkin lymphoma, histiocytic and dendritic cell neoplasms, and spindle cell lesions. Histologic evaluation of the hematoxylin and eosin-stained tissue section remains the cornerstone of analysis, and further studies should be dictated by the morphologic differential diagnosis. Using the morphologic impression as the starting point, virtually all diagnostic problems involving hematolymphoid neoplasms can be resolved by a combination of immunophenotyping (paraffin section immunoperoxidase stains and/or flow cytometry) and/or molecular genetics studies.
