Abstract

Severe chronic venous insufficiency (CVI) is characterized by reflux through the superficial venous system as well as the deep venous system. These phenomena are found in concert with perforating vein outflow. A sizable fraction of patients with chronic venous insufficiency has experienced deep venous thrombosis and some carry persistent venous occlusions. Reports from the Subfascial Perforating Vein Registry have emphasized the importance of such venous obstruction in failures of perforator vein surgery. Surgical approaches to correcting the pathophysiology of venous obstruction have been disappointing. Therefore, exploration of endovascular therapy for chronic venous obstruction has certain appeal.
