Current Concepts and Management of Severely Traumatized Eye: Open-Globe Injury with Endophthalmitis

Current Concepts and Management of Eye Injuries

Abstract

Posttraumatic infectious endophthalmitis is a rare but very severe complication of ocular trauma. Its occurrence aggravates visual prognosis of traumatized eyes. The US Eye Injury Registry reports showed that 3.4 % cases of open-globe injuries are associated with endophthalmitis (Danis 2002). Despite advances in the use of intraocular antibiotics, infection resulting from penetrating ocular trauma continues to present a clinical challenge. It remains an important cause of visual failure following open-globe injuries and may complicate seemingly benign injuries such as small, self-sealing corneal laceration without associated intraocular damage. Microbial inoculation of the aqueous fluid and vitreous gel with subsequent proliferation of these organisms yields a vigorous immune response. Optic nerve, ciliary body, retina, and other ocular structures and also invading pathogens release toxins that cause damage to ocular structures. Prompt antibiotic treatment is recommended by ophthalmologist when traumatic endophthalmitis is suspected (Meredith 1999; Soheilian et al. 2001).
