Abstract
This chapter focuses on the psychological treatment of adolescents with nonbipolar, nonpsychotic depression, e. g., major depressive disorder (MDD) or dysthymia (DY), as specified in the Diagnostic and Statistical Manual of Mental Disorders, third edition— revised (DSM-III-R) (American Psychiatric Association, 1987). Although bipolar disorder is known to occur in adolescence, albeit at a very low prevalence (Bashir, Russell, & Johnson, 1987; Carlson, 1982; Lewinsohn, Hops, Roberts, Seeley, & Andrews, 1993a), the available evidence suggests that its etiology is primarily biological or genetic or both (Medlewicz, 1985).
