Abstract

To explain the mode of action of antianginal vasodilators (Table 4.1), requires first a reasonable hypothesis for the production of angina; secondly, an understanding of the vascular contractile mechanism and its metabolic and neurogenic control; and, thirdly, the way in which these vasodilators interact with their receptors or binding sites to produce clinical effects. On the background of such knowledge, a more rational approach to the principles (Table 4.2) and practice of the use of anti-anginal vasodilators can be formulated. This chapter will deal chiefly with the nitrates and the calcium antagonists (= calcium channel blockers); effects on the circulation as well as on the heart will be emphasized.
