A code is a sequence of symbols used in place of a name to identify an object or a concept. Humans do not think well in codes, but computer systems rely on them, particularly in the design and implementation of databases. This chapter will outline the absolute necessity of understanding the use of codes in the management of clinical data for anyone intending to undertake clinical audit.
