Résumé
L’hyperglycémie de stress, définie par une hyperglycémie transitoire lors de pathologies aiguës chez des patients non diabétiques, est fréquemment observée chez les patients de réanimation (1). La corrélation entre hyperglycémie de stress et mauvais pronostic est largement établie. En 2001, une grande étude randomisée contrôlée monocentrique, chez des patients de réanimation majoritairement chirurgicaux, montrait que le contrôle glycémique strict par insulinothérapie intensive diminuait la mortalité et le taux de complications en réanimation (2). Cependant, les études ultérieures réalisées dans d’autres services de réanimation n’ont pas pu reproduire un tel bénéfice avec ce type de traitement (3–7).
