Résumé
La douleur abdominale aiguë représente 5 % des motifs de recours aux urgences. Elle représente le type même de symptôme où l’urgentiste est amené à gérer un processus diagnostique dans l’incertitude, dans la mesure où aucun élément clinique ou biologique n’est pathognomonique. Le médecin urgentiste est confronté à cette gestion diagnostique à chaque étape du processus de prise en charge dont trois principales peuvent être individualisées. La première consiste à identifier d’emblée les grandes urgences abdominales afin de les traiter sans délais, la seconde est de poser le diagnostic de pathologies nécessitant un traitement chirurgical après quelques informations complémentaires et enfin la troisième est de poser un diagnostic particulier ou d’éliminer les éléments de gravité potentielle pour décider d’une orientation du patient à l’issue de son passage aux urgences [1]. L’intérêt de l’utilisation des biomarqueurs doit être évalué à chacune de ces étapes.
