Endoscopic totally Preperitoneal Ventral Hernia Repair: indications, technique and results

Abstract

The use of prostheses has dramatically reduced the recurrence rate in incisional and primary ventral hernia repair (1). In open surgery, extraperitoneal sublay mesh implantation is generally preferred versus intraperitoneal placement (2), following the same principles as in “giant prosthetic reinforcement of the visceral sac”, described for inguinal hernia repair (3). Endoscopic surgery is a tool to obtain the same operative end-result by minimal invasive means compared to open surgery. Laparoscopic ventral hernia repair (LVHR) with intraabdominal mesh placement has been reported with excellent results, but does not take advantage of immediate mesh fixation by a peritoneal sac. Furthermore, transabdominal laparoscopic incisional hernia repair may lead to various complications due to adhesiolysis and accidental enterotomy (4, 5) and due to late intestinal erosion by the mesh and/or the fixating devices (6). In an attempt to combine the advantages of extraperitoneal mesh implantation with a minimally invasive approach, we describe the technical aspects and our initial clinical experience of a endoscopic totally preperitoneal approach for the treatment of incisional and primary ventral hernias.
