Autologous Fat Grafting in the Breast: Lipofilling

Abstract

The use of autologous fat for correction of body contour defects is not new. The first descriptions date from the end of the nineteenth century, and in the twentieth century this procedure became widely used, mainly in surgical procedures of the hands and the face. The use of this technique in the breast began to be considered in the 1980s, stimulated by the advent of liposuction for aesthetic purposes.

Review article of the American Society of Plastic Surgery regarding the applicability of lipofilling in the breast. It deals with different techniques, effectiveness and safety evidence. The ASPS authorizes the accomplishment of lipofilling provided that informed consent is made. In patients at high risk for breast cancer or with a history of breast cancer, although there is no negative evidence, it suggests that this type of patient be conducted within clinical studies.

Breast. 2011;20(4):351–7. Review article on basic research on the interaction between adipocytes, ADSC, normal cells and tumor mammary cells. There are factors that associate ADSC with the stimulation of tumor cells and others that would provoke inhibition effects. Interaction in humans should be investigated.

Systematic review of 28 in vitro experimental studies and animal models evaluating the interaction between ADSC and cancer cells. All studies demonstrate accelerated growth of tumor cells, as well as faster growth of induced breast tumors in the models and metastasization.

Systematic review of more than 100 articles, 18 prospective clinical studies and cohorts of patients with a history of breast cancer undergoing lipofilling were selected. There was no increase in recurrences in the lipofilling group. Despite the encouraging result, there are no prospective randomized studies. Avoid lipofilling in patients at high risk for local recurrence must be considered.
