Extraperitoneal Hysterectomy-Total Pelvic Peritonectomy Combined with the Segmental Resection of the Rectosigmoid

Abstract

Surgery for ovarian cancer is originally very invasive because of the possibly extensive spread of the tumor and advanced or recurrent disease requires even more super-radical surgery. In this kind of surgery, maintaining surgical clearance is extremely important to prevent future recurrence of disease and for this means, a much more extensive resection is often required. These must undergo not only the initial resection, but also require reconstructive surgery to compensate for large post-resection defects. In advanced or recurrent malignancy cases, even if complete resection is thought to have been achieved, adjuvant therapy is required in many cases. Super-radical procedures are directly related to delayed recovery and more seriously, the delay of postoperative therapy. Laparoscopic surgery is a well-known minimally invasive strategy. However, the feasibility and efficacy for very complicated laparoscopic procedures is yet to be demonstrated and remains controversial. By performing radical surgery laparoscopically, we aim to offer patients a complete, minimally invasive alternative that will allow for early recovery and the implementation of other required therapies. Our purpose is to expand the radicality of laparoscopic hysterectomy for more complete resection while keeping the procedure less invasive by introducing reconstructive techniques along with our extraperitoneal technique.
