Abstract

Adolescents with congenital heart disease (CHD) are vulnerable to lapses in care during the period between leaving pediatric and entering adult CHD care and also prior to transfer. In this chapter, we discuss the best practices in establishing transitional care for CHD, specifically (1) transition programs, including education of adolescents, (2) transition models, (3) transition resources external to the clinical setting, including the role of technology, and (4) future directions for transitions in CHD. To avoid lapses in care and optimize young adults’ knowledge of their heart and health-related topics, transition programs should be instituted as a routine part of care for all adolescents with CHD.
